Pre-graduation degree audit

Who uses this form?

Students who have less than 20 remaining credits OR students who are in their second to last semester.

Form purpose

As many students have learned utilizing the STARS degree audit is often confusing and misleading. The purpose of this pre graduation degree audit is to make sure you are on the same page as the academic advisor. Students need to have a realistic idea of what their remaining requirements are, as well as a clear plan to complete these requirements.

Directions

When you have registered for your second to last semester, OR you have only 20 remaining credits, it is time for you to do your degree audit.

  • Use your plan of study to determine which classes you have remaining. (Make sure you list general education classes you are missing too. )
  • List ALL remaining classes on this form, adding how many credits they are. (Including classes in progress)
  • If you have not completed the CRITICAL THINKING exam at this time, schedule your exam immediately.
  • Make sure you sign the form.
  • Submit the form to the ET advisor for final approval. 

Independent study proposal

Who uses this form?

Students who would like to gain permission to take an independent study (ET 4990, EET 4990, MCT 4990, MIT 4990, ET 7990).

Form purpose

This form serves as a contract between the student and the faculty member to determine the requirements for independent study. Like a syllabus this form sets the expectations for the project, and determines how the student will be assessed and graded.

Directions

First come up with an idea of what you want to study and contact a faculty member to see if they will agree to work with you.

  • Fill out the form with your faculty advisor. This form also serves as an outline for your first meeting with your faculty advisor.
  • Make sure you are clear about the topic of study and how you will be formally graded.
  • Set up a timeframe for you and your advisor to meet.
  • Make sure you and your faculty advisor sign the form.
  • Submit the form to the ET advisor for final approval.

Construction management internship approval form

Who uses this form?

Students in the Construction Management Program who need permission to do an internship (CMT 4200)

Form purpose

This form is to track and make sure students are meeting the requirements of the Construction Management Internship.

Directions

First, secure an internship position within the field of construction management.

  • Fill out the form with your with your supervisor. This form also serves as an outline for your first meeting with your supervisor.
  • Make sure you are clear about what you will be doing on the job.
  • Make sure you and your supervisor sign the form.
  • Take the form to the Construction Management Program Director for approval, once the internship is approved submit to ET advisor.

Construction management internship waiver

Who uses this form?

Students in the Construction Management program who need permission to use a previous job or internship experience for the Construction Management internship (CMT 4200)

Form purpose

This form is to allow students to utilize a previous experience as their CMT internship. Note that if your previous experience is not approved you will be required to do an additional internship.

Directions

Before filling out this form, talk to the CMT program director to see if your previous experience would qualify as a CMT internship.

  • Fill out the form as completely as possible.
  • Make sure you describe your previous experience 
  • Make sure you sign the form.
  • Take the form to the Construction Management Program Director for approval, once the internship is approved submit it to ET advisor.

Prerequisite waiver

Who uses this form?

Students who wish to gain permission to take classes out of order or as co-requisites when prerequisites are required. This is most likely to be granted when a student needs to schedule the classes as corequsites.

Form purpose

This form is to allow students to adjust the order they take classes to accommodate special circumstances. Prerequisites are in place to ensure that students have the foundational information before completing higher level classes. Waiver of a prerequisite is rare and should be used sparingly.  Students must talk to the ET advisor to determine another solution before attempting to waive a prerequisite.

Before filling out this form, talk to the ET advisor and course instructor.

  • Fill out the form as completely as possible.
  • Take the form to the course instructor for permission. (It is best to do this before the first day of class.)
  • Make sure you and the course instructor sign the form.
  • Submit the form to the ET advisor for final approval.

Note: Prerequisites present material which is considered to be foundational to the subsequent courses, and therefore it is not recommended that students take courses out of sequence. If permission is granted in a special circumstance, please do your best to ensure that you are taking steps to be successful in this course, and see your professor or academic advisor at the first sign of academic distress.

This form does not replace a class! IF the mentioned prerequisite is also on your plan of study, you still must take it before degree certification.

Who uses this form?

Students who wish to extend the length of a class so that they are able to complete the course requirements.

Form purpose

This form provides a written record of what needs to be finished in order to earn a grade in a class. The university has established specific guidelines for this which are printed at the top of the form.

  • Read the top portion of this form to make sure you understand the university requirements for an incomplete course.
  • Discuss with your course instructor what must be done to complete the course and fill out the form.
  • Make sure you and the course instructor sign the form.
  • Submit the form to the ET advisor for final approval.
